Schedule H1 Register Format

Short answer: Rule 65(3) of the Drugs and Cosmetics Rules, 1945 says every sale of a Schedule H1 drug must be written in a separate register at the time of supply, with the prescriber's name and address, the patient's name, the drug's name and the quantity supplied, and the register must be kept for three years and be open for inspection.

What must a Schedule H1 register contain?
Five things, according to Rule 65(3): the name and address of the prescriber, the name of the patient, the name of the drug, and the quantity supplied. The rule also says when — at the time of supply, not at the end of the day — and for how long: three years.
The template adds four columns the rule does not ask for: a serial number, the date, the batch number and the pharmacist's signature. They are there because an inspector reading the register, or you tracing a recalled batch, will want them. They are marked separately so nobody mistakes them for legal requirements.
Which drugs go in the Schedule H1 register?
Only the drugs listed in Schedule H1, which was added by notification GSR 588(E) of 30 August 2013. It covers certain newer antibiotics, certain habit-forming drugs and anti-TB drugs. The list changes by notification, so check the current one on the CDSCO website rather than a copy printed years ago.
Can the Schedule H1 register be kept on a computer?
The rule asks for a separate register with those details, kept for three years and open for inspection; it does not name paper. Whether a digital register satisfies your state's drug inspector is a question to put to your licensing authority before you give up the book.
